Mental Health Outreach Worker/Case Manager

Job Summary

Coordinate service support for clients transitioning from prison or jail by providing direct outreach to ensure health and basic needs are met. Complete initial and updated Needs and Service Plans, then meet with each client one-on-one for 30-60 minutes to discuss goals, placement, or discharge. Facilitate weekly group sessions, plan in-house activities, and transport clients to mandated court appointments while speaking on their progress to the court. Locate placements, conduct preplacement interviews, and write progress notes based on all interactions. Manage appointments, communicate with the treatment team, and report to the crisis response team as needed. Attend weekly team, clinical, and one-on-one meetings. Assist in barrier removal and community reintegration via public outings to ensure successful discharge into lower-level care facilities.

Required Qualifications

  • Experience in mental health outreach or case management
  • Ability to provide direct outreach to clients
  • Knowledge of community reintegration processes
  • Experience with discharge planning
  • Ability to facilitate group sessions
  • Experience with court systems and mandated court cases
  • Ability to transport clients to appointments
  • Experience with Needs and Service Plans (NSP's)
  • Ability to write progress notes
  • Experience with preplacement interviews and functional capability forms
  • Ability to communicate with treatment teams and county case managers
  • Experience with crisis response
