Job Summary
Case Manager/Mental Health Counselor for The Doe Fund's Webster permanent supportive housing program serving homeless men and women living with HIV/AIDS. Responsible for providing supportive case management and counseling, maintaining a caseload, developing and implementing individual Service Plans, ensuring residents are linked to healthcare providers and benefits, providing counseling and crisis intervention, assisting with rent payments and budgeting, maintaining records, coordinating with community services, and facilitating groups to promote independent living skills. Requires a Master’s Degree in Social Work or related field and licensure (LMSW or LMHC); minimum 1 year experience with the homeless/HIV/AIDS population and at least 5 years in the mental health field. Location: Bronx, NY (US); on-site program operations.
Required Qualifications
- Master's Degree in Social Work or related field
- License in Clinical Social Work (LMSW or LMHC) required
- At least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness
- At least 5 years of experience in the mental health field
