Job Summary
Provide case management and mental health counseling to residents in a supportive housing program; develop treatment plans; conduct assessments; coordinate with community agencies; lead monthly tenant meetings; maintain electronic records and progress notes; coordinate care with collateral contacts (family, providers, ACS/APS, Schools); work under Clinical Supervisor to promote optimal mental health and client functioning; engage adults and families, educate on diagnoses and treatments, and connect clients with community resources to support housing stability and well-being.
Required Qualifications
- Master’s degree in social work, mental health counseling, psychology, sociology, etc.
- 2-3 years’ experience in a residential setting
- Familiarity with NYC Supportive Housing; mental health, substance use, HIV/AIDS, or related field
