Job Summary

Co-facilitate individual, parent, and family therapy sessions while building strong relationships with children and families at risk of psychiatric hospitalization. Provide case management, advocacy, and care coordination by connecting clients to community resources and collaborating with schools, DCF, and healthcare providers. Support discharge planning and successful transitions to ongoing care, ensuring children ages 5–18 remain safely at home. Complete timely documentation and actively participate in supervision and team meetings as part of a two-person IICAPS team.

Required Qualifications

  • Bachelor's Degree in Human Services, Social Work (BSW), or related field
  • Valid driver's license and reliable transportation

Desired Qualifications

  • 3+ years of human services experience
  • Experience working with children, adolescents, and families
  • Strong communication, organization, and problem-solving skills
  • Cultural competency and experience working with diverse populations
  • Bilingual Spanish preferred
