Mental Health Counselor - Crotona Residence

Mental Health Counselor for Crotona Park residence providing case management and counseling services to residents living with HIV/AIDS in a permanent supportive housing program. Responsibilities include maintaining caseloads and records, conducting and reviewing assessments, developing and implementing individual Service Plans, linking residents to healthcare providers, providing counseling and crisis intervention, assisting with benefits, liaising with hospitals, treatment programs, HRA, and other community services, facilitating groups and activities to promote independent living, monitoring rent payments and budgeting, conducting unit inspections, educating residents on mental health approaches, compiling resources on mental health information and treatment strategies, attending ongoing training, and assisting with HASA reports. Ideal candidate holds a MasterA0 Degree in Social Work with an LCSW, has at least one year of relevant experience with the target population and at least five years in the mental health field, and possesses knowledge of NYC Metro benefits, HIV/AIDS, psychiatric diagnoses, and substance use issues; bilingual ability is a plus.
