Mental Health Consultant (LICSW)Provides solution-focused therapy to patients independently and/or with mental health providers; conducts diagnostic assessments; establishes and updates treatment goals; completes discharges with program participants; maintains accurate patient records; collaborates with Primary Care/Women’s Health teams and community supports; coordinates care across primary care, specialties, and mental health service lines; triages crises with same-day and short-term access to therapy; screens for basic health needs and aligns services; may consult, refer, and coordinate interventions for child or adult abuse, domestic violence, or sexual abuse; supports patient goals and provider communication about plan of care.
