Job Summary
Conduct case management for youth using the MST-FIT model, providing aftercare support during transitions and crisis intervention throughout the week, including evenings and weekends. Perform field work requiring driving residents and conducting weekly documentation within 48 hours of occurrence. Prepare MST-FIT engagement court reports for permanency hearings and attend mandatory trainings, supervision, and group consultations. Ensure ongoing collaboration across MST and ITM program staff while adhering to ACS and OCFS regulations. This role supports the social service department's resident transition efforts at our Rockaway Park campus and Richmond Hill detention residence.
Required Qualifications
- Minimum MSW degree with licensure preferred
- Minimum of 1 year of experience working in child welfare with at-risk youth
- Valid NYS Driver's License
- Professional, mature, and dependable with a bias for action
- Ability to assess and engage youth and families
- Physical ability to travel throughout the community, securing resources and services, while adhering to the MST 120-day targeted length of treatment model
- Willingness to adjust schedule as needed to ensure service needs are met
- Must be able to lift 50 lbs
