Job Summary
Mental Health Coach (MST-FIT) delivers case management for at-risk youth and families using MST-FIT model, conducts with youth/families through virtual and in-person sessions, performs administrative caseload activities per ACS/OCFS regulations, and may require evening hours and home visits. Responsibilities include crisis intervention, weekly documentation within 48 hours, collaboration across MST/ITM programs, preparing court reports and attending trainings, with travel across Rockaway-area sites for service delivery. Requires MSW degree (licensure preferred), 1 year child welfare experience, valid NYS driver’s license, and ability to travel within the community; hybrid/in-field schedule with home visits and travel to multiple locations.
Required Qualifications
- Minimum MSW degree with licensure preferred
- Minimum of 1 year of experience working in child welfare with at risk youth
- Former Case Management experience is a plus
- Valid NYS Driver's License
- Physical ability to travel throughout the community
- Willingness to adjust schedule as needed to ensure service needs are met
