Job Summary
Case Manager – ODR (ICMS) works in the Intensive Case Management Services team to provide field-based outreach, housing navigation, and service coordination for adults experiencing homelessness and high-need conditions across Los Angeles County. Responsibilities include outreach with DHS, sheriff’s department, Probation, and DMH to identify housing options; assess eligibility for permanent supportive housing; assist with rental subsidies and housing searches; coordinate mental health, medical, and recovery services; develop and update individualized service plans; maintain compliant documentation in the electronic health record; and ensure tenancy stability. Requires at least 1 year of relevant experience and a bachelor’s degree (or equivalent experience); travel throughout LA County; strong collaboration with county partners; and a drive to support housing stabilization and community reintegration.
Required Qualifications
- Bachelor’s degree in social work, psychology, or related mental health field, or equivalent experience
- Experience with homelessness, mental illness, chronic medical conditions, and substance use disorders
- Knowledge of housing systems, landlord-tenant rights, eviction prevention, and affordable housing
- Ability to use electronic health records for documentation
- Valid California Driver’s License, clean driving record, and insurability
- Live Scan fingerprint clearance
- COVID-19 vaccination and booster as required by policy
