Job Summary
Case manager embedded in Catholic Charities’ permanent supportive housing program to manage a caseload of clients with mental health needs, developing individualized, trauma-informed, person-centered plans; co-facilitating case consultations; coordinating referrals to external providers; maintaining complete documentation and confidentiality; supporting housing retention and benefits; and collaborating with an interdisciplinary team to promote psychiatric stability, independent living skills, and community integration.
Required Qualifications
- Master’s degree in social work, counseling, or psychology, with eligibility for Mental Health Practitioner or Professional licensure in the State of Minnesota (e.g., LGSW, LICSW, LMFT, LPCC)
- At least two years of supervised experience providing services to adults or children with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities, plus training in mental illness recovery, mental health de-escalation, co-occurring mental illness and substance use, and psychotropic medications and side effects
- Experience providing licensed mental health care directives and developing treatment plans
- Valid driver’s license; may be required to transport clients
- Ability to work with diverse and low-income populations, including individuals experiencing homelessness, unstable housing, or mental illness
