Job Summary
Mental Health Case Manager coordinates healthcare services for children in the Baltimore City foster care system, partnering with BCDSS and foster families to ensure timely medical and mental health care, develop healthcare plans, navigate HealthChoice, and support caregivers and child welfare staff through education and resources. The role requires coordinating with a multidisciplinary team, documenting activities in BCDSS and HCAM databases, and utilizing multiple data systems to verify and track information, with emphasis on organizational, communication, and culturally sensitive care.
Required Qualifications
- A Master’s Degree in Social Work or any Human Services field from an accredited college/university
- One-year related experience working with children/youth
- Relevant experience providing services to families and children, preferably in a public health/community setting
- Knowledge of MS Word, Excel, Outlook and SharePoint (365)
- Strong organizational, communication and problem-solving skills
- Sensitivity to cultural and socioeconomic characteristics of population served
- Housing First philosophy knowledge preferred; support required
- Excellent oral and written interpersonal and communication skills
- Ability to meet deadlines and multi-task in a demanding workload
