Job Summary
Mental Health Case Manager providing case management and supportive counseling for identified children with mental health diagnoses and their families in Montgomery County. Responsibilities include completing assessments and treatment plans, delivering home/school/hospital/community visits, coordinating services with agency providers, documenting via IPAD, and achieving billable hours. Travel throughout Montgomery County is required; role involves on-call 24/7 crisis support and adherence to Trauma-Informed Care core values (Safety, Trustworthiness, Choice, Collaboration, Empowerment).
Required Qualifications
- 12 credits in coursework related to Human Services, Psychology or related field
- One or more years of direct clinical experience working with children, youth and families
- Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week
