Job Summary

Provide supportive care to identified children with mental health diagnoses and their families in Bucks County by navigating complex systems, coordinating resources, and delivering face-to-face counseling. Conduct daily home, school, hospital, and community visits to diffuse crises and prevent unnecessary emergency or inpatient use. Complete comprehensive assessments, treatment plans, and written strength assessments for each client on a caseload of up to 30 individuals. Utilize provided technology to document outreach and connect consumers to behavioral health and social service agencies to ensure no gaps in care. Participate in weekly team meetings and provide on-call support for 24/7 mental health crisis rotation. Manage a varied schedule Monday-Friday between 8:30am and 8:30pm, including three required evenings per week, while adhering to 5 core values of Trauma-Informed Care.

Required Qualifications

  • 12 credits in coursework related to Human Services, Psychology or related field
  • One or more years of direct clinical experience working with children, youth and families
  • Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week
  • Must be able to travel throughout Bucks County to meet face to face with clients, families, schools, providers etc. as based on client needs
  • Provide on call support for 24/7 mental health crisis support in a rotation with other program staff
