Memory Care Licensed Practical Nurse (LPN)

Assist residents with activities of daily living, including bathing, dressing, meal service, and medication administration in accordance with Tennessee rules. Monitor resident health by visiting daily to evaluate physical and emotional status, checking vital signs, recording weights, and notifying physicians of accidents or condition changes. Administer prescribed medications, verify resident identity, document treatments, and ensure adequate supplies and equipment are available. Admit, transfer, and discharge residents as necessary while communicating updates to families and staff. Supervise team members to carry out care plans and participate in in-service training to maintain professional competence. Treat residents with dignity and confidentiality, promptly reporting any complaints or grievances to the Assisted Living Director.
