Memory Care Leader (QMA)

Oversee daily operations of the Legacy Lane memory care program, ensuring alignment with dementia care best practices and maintaining a dementia-safe environment through daily resident room rounds. Lead initiatives to educate campus staff on dementia topics, serve as the Dementia Champion, and support efforts to maintain appropriate resident populations per move-in/move-out criteria. Conduct interviews for potential Legacy team members and develop personalized Service Plans for residents during Resident First Meetings. Maintain open communication with the staffing coordinator to ensure consistent specialized dementia care staffing. Provide care within the scope of a Qualified Medication Aide. This role requires current CNA and QMA certifications and 1-3 years of relevant experience.
