Memory Care Leader (LPN)

Oversee daily operations of the Legacy Lane memory care program, ensuring alignment with dementia care best practices and maintaining a dementia-safe environment through daily resident room rounds. Lead initiatives to educate campus staff and families on dementia topics while serving as the Dementia Champion. Collaborate with leadership to manage resident population levels, conduct interviews for new team members, and maintain open communication with the staffing coordinator for specialized care. Participate in Resident First Meetings to develop personalized Service Plans for memory care residents. Provide care within the scope of a Licensed Practical Nurse.
