Job Summary
Memory Care Unit Director (LPN) oversees clinical operations, resident care services, and day-to-day functions of the Memory Care neighborhood at Firwood Gardens. Serves as clinical leader, mentor, and resource to caregiving staff; collaborates with Executive Director, Regional Nurse Consultant, physicians, families, and interdisciplinary teams to promote resident outcomes and quality of life. Coordinates resident care services, assessments, service plan reviews, and monitoring of condition changes; participates in admissions, transitions, and documentation in compliance with Oregon regulations. Provides supervision and coaching to caregivers and staff, participates in hiring, onboarding, training, and performance management; supports medication oversight, infection prevention, and adherence to regulatory requirements. Promotes person-centered dementia care, facilitates meaningful programming, supports behavioral interventions, ensures readiness for state surveys and audits, and maintains compliant resident records.
Required Qualifications
- Active Oregon Licensed Practical Nurse (LPN) license
- Experience working with older adults, dementia care, memory care, assisted living, or long-term care population preferred
- Ability to effectively supervise and support staff
- Strong clinical assessment, communication, and organizational skills
- Ability to maintain confidentiality and professionalism
- Current CPR/First Aid certification or ability to obtain upon hire
- Ability to successfully complete required background checks
