Memory Care Coordinator (RN/LPN) Registered Nurse/Licensed Practical Nurse

Assess residents' cognitive, physical, psychosocial, and behavioral needs and participate in developing, implementing, and reviewing individualized care plans. Monitor resident progress, recommend care plan revisions, and assist in identifying behavioral triggers to implement non-pharmacological interventions. Collaborate with the interdisciplinary team, attend care conferences and family meetings, and administer medications and treatments per physician orders. Support the delivery of evidence-based dementia care practices, monitor intervention effectiveness, and serve as a resource for memory care best practices throughout the facility. Coordinate memory care programming, collaborate with Activities and Therapy Departments, and provide education to families regarding disease progression and care approaches. Work in both office and in-unit settings, often with residents, staff, and visitors, and participate in the facility's on-call rotation schedule.
