Job Summary
Memory Care Coordinator leads resident life in the Memory Care neighborhood through a comprehensive, person-centered approach and collaborates with nursing staff to provide continuity of care. Responsible for developing and implementing dementia-focused service plans, coordinating move-in planning and life-story work, and delivering caregiver guidance and ongoing education. Builds meaningful programs and events, coordinates a week-long calendar (including evenings), and promotes resident engagement with residents and families. Strong interpersonal, organizational, and communication skills are essential to plan, develop, organize, and implement programs and to adapt to changes. Preference for memory care course completion and experience delivering leisure/recreational programs for individuals with dementia. This role reports to the Memory Care Manager or Executive Director.
