Job Summary
Medication Use Safety Clinical Coordinator provides comprehensive collaborative pharmaceutical care to ensure safe, effective, and cost-conscious drug therapy. Responsibilities include active participation in multidisciplinary rounds, timely medication order processing, reporting medication incidents, leading unit/team performance improvement, and coaching/education of pharmacy staff and trainees. Supports residency programs, precepts rotations, and contributes to scholarly activities such as publications or poster presentations. Demonstrates leadership to advance pharmaceutical care within the department and organization.
Required Qualifications
- Bachelor's Degree or Doctorate in Pharmacy
- ASHP-accredited PGY-1 and PGY-2 Pharmacy Residency Program OR PGY-1 plus 2 years of practice OR 3+ years of practice in area of focus
- Licensed Pharmacist - State Board of Pharmacy in state of primary practice site within 120 days of start date
- Basic Cardiac Life Support (BLS)
- Preferred Licensure and Certification: Neonatal Resuscitation Program (NRP), Pediatric Advanced Life Support (PALS), Advanced Cardiac Life Support (ACLS)
