Job Summary
Obtain and record vital signs, reporting deviations to the supervising nurse per the Individual Service Plan. Assist residents with personal care including bathing, grooming, dressing, toileting, incontinence care, nutrition, ambulation, transfers, positioning, and medication reminders. Follow daily task sheets, communicate resident status changes to supervisors, and maintain safe, clean environments while documenting progress. Utilize standard precautions, encourage resident engagement, and uphold HIPAA confidentiality. Attend biweekly in-service meetings and mandatory trainings. Perform additional duties as assigned.
Required Qualifications
- High school diploma or GED (or equivalent experience)
- Minimum six months caregiving experience, preferably with elderly residents
- Completion of required medication management course
- CPR certification within 90 days of hire
- Professionalism and ability to handle emotionally charged situations
- Ability to work collaboratively in a multidisciplinary team
