Job Summary
Under the direction of a pharmacist, the Medication Management Technician is responsible for collecting and electronically documenting medication and allergy histories for new admissions, primarily stationed in the Emergency Department, and for ensuring accurate and timely medication reconciliation. Major duties include interviewing patients and/or caregivers to obtain complete medication lists (prescriptions, OTCs, vitamins, supplements), recording details (name, dosage, form, strength, timing, source) in the electronic medical record, obtaining and documenting allergy history, clarifying uncertainties from multiple sources (pharmacy records, clinic notes, prior encounters), and consulting with a pharmacist for unresolved issues. The role emphasizes collaboration with pharmacists, nursing staff, and providers, along with adherence to standards of respect and patient-centered care.
Required Qualifications
- High School diploma or equivalent
- Massachusetts Registered Pharmacy Technician License
- Minimum age of 18
- Completion of at least 500 hours as a pharmacy technician trainee
- National Pharmacy Technician Certification (Preferred)
- Verbal and written communication skills
- High level of professionalism
