Job Summary
Under the direct supervision of a Pharmacist, the Medication History Specialist will determine the patient’s prior to admission medication list, including strength, dosage, frequency, and last dose, and will communicate with the Pharmacist to evaluate changes to inpatient medications. The role emphasizes accurate medication history documentation, patient interviewing, EMR data entry, HIPAA/compliance, and coordination within hospital pharmacy settings (inpatient/outpatient) to ensure patient safety.
Required Qualifications
- High school diploma or equivalent
- Minimum of 1 year of experience in a healthcare, pharmacy, medical office or clinical support role
- Ability to work independently while following established procedures
- Ability to maintain patient confidentiality and comply with HIPAA regulations
- Professional demeanor with a compassionate, patient-focused approach
