Job Summary
Certified Pharmacy Technician II who collects and documents patients' home and outpatient medication histories for hospital admissions and surgical care, conducts interviews with patients and families, contacts outpatient pharmacies and long-term care facilities to verify medication information, enters the good faith effort medication list into the electronic medical record for pharmacists, providers, nurses, and other healthcare team members to support treatment planning, medication orders during hospitalization, and post-discharge care, and supports effective communication and coordination within the care team. Requires 2+ years of retail experience, Indiana license (or intern/extern), and Pharmacy Technician National Certification (unless enrolled in accredited college of pharmacy); emphasizes strong organizational, multitasking, and customer-service skills along with solid written and spoken communication and proficiency with typing and Microsoft Office.
Required Qualifications
- High School Diploma or GED
- Indiana Pharmacy Technician License OR intern/extern license if enrolled in accredited college of pharmacy
- Pharmacy Technician National Certification required unless enrolled in accredited college of pharmacy
- 2+ years relevant experience preferred
- 3-5 years recent retail experience
