Job Summary
Obtain accurate, comprehensive medication history by interviewing patients and communicating with outside sources to determine all current and active prescriptions. Evaluate medication adherence based on patient interviews and accurately document the medication history for physician reconciliation. This Medication History Technician supports admitted patients and providers within the Clinical Pharmacy Services team at Northwest Hospital, a LifeBridge Health facility serving Baltimore County communities.
Required Qualifications
- Pharmacy Technician License
- Certified Pharmacy Technician (PTCB)
- Pharmacy Technician (AAH)
- Nationally Registered Certified Pharmacy Technician
- 3-5 years related experience
