Job Summary
Coordinate care across patients, providers, pharmacies, and payors to ensure timely, affordable, and well-coordinated pharmacy care for those with complex medication needs. Identify opportunities to simplify medication schedules, engage patients in ongoing support, and verify that financial and billing pathways remain clear and accurate. Manage the Medication Care Program Team's mission to remove access barriers and keep therapies aligned using technology, communication skills, and clinical insight. Work Monday through Friday, 8:00 a.m. to 4:30 p.m., onsite at Parkview Integrated Pharmacy or Medical Park 11 Pharmacy, with a potential transition to hybrid.
