Job Summary
Medicare Wellness Nurse collaborates with the primary care team to identify Medicare patients appropriate for Annual Wellness Visits (AWV) and conducts initial and annual preventive care visits. Responsibilities include assessing follow-up needs, connecting patients with community resources, and implementing the standards of care for the Medicare annual wellness program, all with empathy for patients and families.
Required Qualifications
- Graduate from an approved school of Nursing.
- Current RN license with North Carolina State Board of Nursing or Compact License recognized by the North Carolina State Board of Nursing.
- One year of nursing experience required, 2 years preferred.
- Must maintain current AHA BLS certification.
- Demonstrate knowledge of current trends and techniques in nursing.
