Job Summary
Field RN Home Health Visits for Activa Home Health in Melbourne, FL. Provide safe and effective nursing care per Plan of Care, coordinate with Team, communicate changes in patient status, and perform duties including wound care, infusion, and catheter care. Demonstrates empathy, attention to detail, and urgency in patient care; collaborates with Supervisor/Manager and Case Manager. Requires Florida RN license and experience in home health; background screening required. This role covers Sebastian and Barefoot Bay areas and may involve part-time shifts with focus on high-quality patient care.
Required Qualifications
- Current RN license, state of Florida
- Wound care and infusion skills
- Excellent communication skills; team player; interpersonal skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
- 1 year of Home Health experience preferred
Additional Requirements
- Care Provider Background Screening Clearinghouse background check required
