Job Summary
Perform comprehensive physical therapy evaluations for Medicare home health and Hospital at Home patients, assessing strength, balance, gait, and functional mobility. Develop and implement individualized, evidence-based treatment plans to address functional decline from acute illness or injury. Provide gait training, balance interventions, and mobility progression while educating patients and caregivers on safe transfers, fall prevention, and assistive device use. Monitor patient progress, complete accurate OASIS documentation, and collaborate with physicians and interdisciplinary teams to support safe transitions. Requires an active Florida PT license, DPT degree, and CPR certification.
Required Qualifications
- Active Florida Physical Therapist (PT) license
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong clinical reasoning, communication, and time-management skills
Desired Qualifications
- DPT preferred
- Medicare home health experience
- OASIS experience
- Acute care or hospital-based physical therapy
- Hospital at Home, home health, or transitional care experience
- Experience treating medically complex patients
- Familiarity with EMR systems
