Job Summary
RN to provide skilled nursing care to senior residents in a long-term care and rehabilitation setting, including wound management, IV therapy, catheter/tube care, pain management, and coordination with an interdisciplinary care team. Responsible for developing and implementing individualized care plans, administering medications and treatments, documenting in electronic medical records, mentoring CNAs, educating patients and families on diagnoses and care routines, promoting infection control, fall prevention, and adherence to regulatory standards, and supporting discharge planning in a patient-first environment.
Required Qualifications
- Licensed RN (Registered Nurse) in good standing
- BLS/CPR certification typically required
- Experience in long-term care or rehabilitation settings preferred
- Ability to assess patient needs and develop individualized care plans
- Strong documentation skills and EMR proficiency
- Ability to educate patients and families
- Team collaboration and mentorship of CNAs
- Knowledge of infection control and regulatory standards
