Job Summary
Coordinate care for high-risk maternity, newborn, and emergency department patients across the continuum of care. Oversee interdisciplinary activity and discharge planning while assessing and screening individuals from newborn to 100+ years for discharge needs and readmission prevention. Provide referrals for continuing care and locate community resources to align with patient and family healthcare goals. Requires a Bachelor's degree, two years of health or counseling experience, and Ohio licensure. Preferred qualifications include a Master's in Social Work and LISW certification.
Required Qualifications
- Bachelor's degree
- Two years' work experience in a health institution, child welfare institution agency, psychiatric clinic, or family counseling service agency
- Licensed Social Worker in the state of Ohio
Desired Qualifications
- Master's Level Degree in Social Work
- LISW or LISW-S
