Job Summary
Medical Social Worker on a full-time day shift (9:00am-5:00pm, Monday-Friday, including every 4th weekend with Saturday-Sunday coverage) at NewYork-Presbyterian Brooklyn Methodist Hospital. Responsibilities include psychosocial assessments, identifying barriers to safe discharge, coordinating referrals to skilled nursing and rehab facilities, documenting in Epic, handling abuse-related cases, and educating patients/families about post-acute care options and payor issues. Requires MSW and NY LMSW license; preferred experience in acute care, discharge planning, and proficiency with Epic and Allscripts. On-site role with commutable distance to NYP. Salary range $44.16-$53.81/hour.
Required Qualifications
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Master Social Worker (LMSW)
- At least one year of experience in an acute care setting
- Discharge planning experience
- Experience with Epic and Allscripts
