Job Summary
Collaborate with patients, families, healthcare team members, and community organizations to identify needs and resources within the hospital and community. Develop and evaluate effective transition plans that account for client diagnosis, prognosis, treatment courses, past and present services, short-term and long-term goals, provider options, and available health care benefits. Maintain strong advocacy skills throughout the continuum of care to support discharge planning and resource coordination. Requires a Master's degree in Social Work, BLS certification, and one year of acute care and case management experience.
Required Qualifications
- Master's Degree – Social Work
- Basic Life Support (BLS) - Certification - American Heart Association (AHA)
- RQI within 90 days of hire
- Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC)
- 1 year of case management experience
- 1 year of acute care experience
