Job Summary
Educate patients on treatment, health, and wellness choices for those living with kidney disease to enhance quality of life. Conduct psychosocial assessments, participate in care planning and team reviews of patient needs, and manage services to assist individuals in coping with chronic kidney disease and dialysis treatments. Support the social and emotional well-being of patients at Satellite Healthcare while working directly with a multi-disciplinary team to address needs holistically.
Required Qualifications
- Master's Degree in Social Work from an accredited CSWE institution
- MSW
- LCSW/ACSW
- Licensure/LMSW in states with licensure (TX, TN)
Desired Qualifications
- Preferable experience in other health and human services fields
- Medical experience
- One (1) year of work experience in a behavioral or healthcare-related setting
- LCSW (CA)
