Job Summary
Develop a safe and timely discharge plan complying with regulations, conduct family meetings to support decision-making, and lead efforts to obtain charity resources, legal guardianship, and psychiatric referrals. Coordinate patient discharge and continuity of care while providing crisis intervention and educating staff on psychosocial needs. Complete initial screens and comprehensive psychosocial assessments, assist families in understanding treatment options, and collaborate with Risk Management and Quality Management to preclude detrimental outcomes. Demonstrate flexibility in department coverage and adjust practice to acknowledge diversity.
Required Qualifications
- Master's Degree of Social Work degree from an accredited school of Social Work Program
- 1-3 years as graduate student or as a social worker with experience in the field of Clinical Social Work with experience in care transitions
- LMSW - Licensed Medical Social Worker - State Licensure
- Knowledge of accreditation standards, health care regulations, performance improvement and patient safety
- Knowledge of health care law and regulations related to acute care and the immediate post-acute continuum
- Ability to complete a psychosocial assessment, risk factor analysis, treatment planning, and models of intervention

- Ability to create collegial and collaborative relationships internally and externally
- Effective organization, oral and written communication skills, problem solving, program development and computer skills
