Job Summary
Assess patient psychosocial and environmental needs, assist with discharge planning and long-term care arrangements, provide counseling and emotional support to patients and families, and connect patients with community resources and financial assistance programs. Complete required documentation per Medicare/home health standards and participate in care coordination with the interdisciplinary team. This role offers a flexible schedule with mileage reimbursement for compassionate support of home health patients and their families in Florida.
Required Qualifications
- Master's degree in Social Work (MSW)
- Active Florida license (LMSW/LCSW preferred)

- Home health or healthcare experience preferred
- Strong knowledge of community resources
- Excellent communication and organizational skills
