Job Summary
Assess patients and caregivers for biopsychosocial needs, including emotional, social, environmental, and financial factors, to develop individualized treatment plans. Collaborate with interdisciplinary medical teams to coordinate care conferences and monitor service receipt to ensure timely, appropriate support. Provide therapeutic relationships, counseling, and mental health clinical interventions under supervision while functioning in an on-call capacity to meet departmental needs. Report to the Medical Social Work Manager. Master's degree in Social Work or related licensed field required with six to twelve months experience preferred. Increased security clearance involving FBI fingerprinting and CPS registry search is required for roles in Medical Tower II.
Required Qualifications
- Master's Degree in Social Work, Counseling or related licensed eligible field
- Increased Security Clearance Required for Job Roles in Medical Tower II: FBI fingerprinting, criminal background check, and Child Protective Services registry search
Desired Qualifications
- Course work in case management, advocacy, and family systems preferred
- Approximately six to twelve months on-the-job experience preferred
