Job Summary
Provide direct social work service and counseling to patients and families, including crisis intervention and discharge planning for complex cases. Gather data to assess medical, social, and emotional situations, then formulate intervention plans that address social determinants of health and care coordination. Float to other hospital floors or teams as needed by management while actively participating in department activities, staff meetings, and committees. Mentor less experienced social workers and document all assessments, plans, and interdisciplinary communications according to standards.
Required Qualifications
- Master's Degree in Social Work or equivalent
- One (1) year of clinical and/or medical social work experience
Desired Qualifications
- Current CA Licensed Clinical Social Worker (LCSW) license
- Previous experience as a social worker in an acute medical setting
- Knowledge of contemporary behavioral and systems theories relevant to healthcare, end-of-life dynamics and interventions, grief and bereavement counseling, substance abuse identification and interventions, support for victims of abuse, neglect or violence
- Knowledge of community or financial resources for underserved or vulnerable populations
- Bilingual (English/Spanish)
