Job Summary
PRN Medical Social Worker (MSW) provides social work services to home health patients in San Antonio and surrounding areas. Responsibilities include assessing patients' psychosocial status, developing and implementing individualized plans to address social, emotional, and environmental needs, assisting with accessing community resources, providing counseling and emotional support related to illness and caregiving challenges, collaborating with physicians, nurses, therapists, and other members of the interdisciplinary team, participating in care planning and conferences, documenting assessments and progress, educating patients and families about available resources, advance directives, and long-term planning options, promoting patient rights and self-determination, and delivering care independently within an interdisciplinary team framework.
Required Qualifications
- Master's Degree in Social Work (MSW) from an accredited school of social work
- Current Texas licensure as required by state regulations.
