Medical Social Worker (MSW)

Conduct patient assessments and ongoing care for hospice patients while managing casework needs for terminal ill patients and their families. Complete documentation accurately and in a timely manner, communicate with patients, families, and co-workers, and participate in discharge planning and in-service programs. Maintain a valid driver's license and reliable transportation for in-home visits. Requires a Master's Degree in Social Work and current Louisiana license; previous home health or hospice experience preferred.
