Medical Social Worker MSW

Assess the psychosocial status of patients related to their illness and environment, then carry out social evaluations and plans intervention based on findings. Maintain clinical records for all referred patients and provide information and referral services regarding practical and environmental needs. Serve as a liaison between patients, families, and community agencies while maintaining collaborative relationships with organizational personnel. Participate in the development of total plans of care, case conferences, and discharge planning, and assist physicians in understanding significant social and emotional factors. Coverage includes the Peninsula and Southside. Requires a master's degree in social work and one year of healthcare experience.
