Job Summary
Medical Social Worker (MSW/LSW) in home health is needed to support patient care across in-home settings. Responsibilities include conducting psychosocial assessments, developing and reevaluating plans of care, communicating findings to supervisors and physicians, ensuring safety and discharge planning, writing physician orders for visits, and counseling patients and families to meet social work-related needs. Requires a Master’s degree in Social Work, 1+ year healthcare social work experience, licensure as a Social Worker, strong communication skills, willingness to travel up to 50%, and a valid driver’s license. Part-time or PRN position with potential for education and advancement within a supportive team at Elara Caring.
Required Qualifications
- Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council on Social Work Education
- 1+ year of social work experience in a healthcare setting
- Current State License as a Social Worker
- Excellent verbal and written communication skills
- Social work skills as defined as general social work standards
- 50% travel required
- Valid driver’s license and insurance and reliable transportation to perform job tasks
