Medical Social Worker / MSW (Hospice)

Assess psychosocial status of patients and collaborate with interdisciplinary teams to develop personalized care plans. Help patients and families with advance care planning, including discussions about advance directives and comfort care options. Carry out social evaluations, maintain clinical records, and provide information and referral services for practical and environmental needs. Offer individual, family, and group counseling to help patients and caregivers manage grief, fear, and anxiety while assisting families in navigating hospice care challenges. Serve as a liaison between patients, families, and community agencies for financial assistance, legal support, and respite care services. This role requires an MSW with one year of healthcare experience, BLS/CPR certification, and a valid driver's license. Pay range is $35.00-$42.00 hourly for per diem shifts in Yucca Valley, 29 Palms.
