Medical Social Worker (MSW) - Home Health

Job Summary
Provide home-based social services to patients in their homes, assessing psychosocial needs, counseling and connecting patients and families to resources to support recovery and quality of life. Work as part of a home health team to implement the patient’s plan of care, monitor progress toward goals, conduct assessments, document outcomes, and advocate for patients and caregivers. Collaborate with an interdisciplinary team (RN, LPN, CNA, HHA, PT, OT, SLP) to address psychosocial needs, crisis intervention, and community resource planning, while maintaining compliance with home health regulations and prioritizing compassionate, patient-centered care.
Required Qualifications
- Master’s degree in Social Work and active MSW license in Massachusetts
- Minimum of one years of social work experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Strong interpersonal skills, good communicator, empathetic, compassionate and resourceful
