Medical Social Worker (MSW) Home Care

Provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes. Employs counseling skills to help patients/families understand and follow recommendations for care and to restore patient to optimum social and health adjustment. Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network; stays up-to-date on current trends and resources; intervenes in crisis situations using crisis intervention techniques; participates in audits to ensure adherence to laws and guidelines; reports suspected abuse/neglect/exploitation; collaborates with patients and their social support system to implement a care plan; conducts initial psychosocial evaluations; supports ethical practice and patient rights knowledge, including HIPAA.
