Job Summary
Provide psychosocial assessments, counseling, crisis intervention, and information/referral services on a consultation basis. Collaborate with the management team to ensure efficient resource utilization for positive patient outcomes. Employ counseling skills to help patients and families understand care recommendations and restore social and health adjustment. Stay current on trends and research community agency availability for social, emotional, and financial assistance. Intervene in crisis situations using established theory techniques. Identify and record clinical, psychosocial, and financial barriers to smooth healthcare transitions. Interview patients to complete initial psychosocial evaluations and report suspected abuse or neglect cases. Work with patients and support systems to establish and implement care plans. Participate in monthly or yearly audits to ensure adherence to federal guidelines. Requires a Master's degree in Social Work, one year of experience, and ability to work with diverse backgrounds.
Required Qualifications
- Master's Degree in Social Work
- 1 year experience working as a social worker or applicable mental health experience
- Ability to work effectively with people from a broad range of backgrounds
- Ability to adhere to the National Association of Social Workers Code of Ethics
- Knowledge of patient rights and laws relative to those rights, such as HIPAA
- Knowledge of psychological principles and theories
- Must possess a high degree of emotional intelligence, and empathy towards others
- Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems
