Medical Social Worker (MSW)

Conduct patient assessments and ongoing care for hospice patients in Covington, LA, while managing casework needs for terminal ill patients and their families. Complete documentation accurately and in a timely manner, communicate with patients, families, and co-workers, and participate in discharge planning and in-service programs. Maintain a valid driver's license and reliable transportation for in-home visits. Requires a Master's Degree in Social Work and current Louisiana license, with previous hospice experience preferred.
