Medical Social Worker (MSW)

Conduct comprehensive patient assessments and develop individualized Plans of Care for high-acuity, medically-at-risk aging patients in home settings. Identify barriers to care, connect families with community resources, and assist with financial assistance applications and long-term planning. Provide counseling on adjustment, coping, and grief while collaborating with physicians, nurses, and therapists to improve patient outcomes. Complete accurate clinical documentation in accordance with Medicare and federal guidelines, observing and communicating changes in patient condition. Schedule: Per diem, 1 day a week, 5-6 patients weekly. Territory covers Macedonia, Twinsburg, Parma, and Cleveland. Requires current MSW license, Master's degree, and one year of acute care experience.
