Job Summary
The Medical Social Worker collaborates with an interdisciplinary care team to provide assessments, advocacy, and comprehensive care coordination for high-risk patients and their caregivers. Responsibilities include proactive assessments, telephonic check-ins, and in-person visits; identifying goals with patients and support persons; utilizing community resources and referrals; navigating insurance benefits with the PRM to maximize plan benefits; delivering longitudinal and episodic care coordination to empower patients to manage health conditions; educating patients and families on supportive care options, including advance care planning and end-of-life care; providing psychoeducation on life stage and disease-specific issues; collaborating with care teams to develop solutions for complex psychosocial dynamics; documenting interventions; and mentoring Community Health Workers. The role reports to the Practice Manager and is 40 hours per week, with competitive compensation and benefits as part of CVS Health. This full-time position emphasizes quality, safety, and patient-centered care for Medicare-eligible populations within Oak Street Health Centers in the Bronx. Qualifications emphasize CSWE-accredited MSW, licensure, multilingual capabilities, and healthcare experience.
Required Qualifications
- Master of Social Work degree required from a CSWE-accredited school
- Licensed in the state where OSH Center is located (minimally)
- Spanish, Polish, Russian, or other languages spoken by communities served (where necessary)
- Proficient computer skills (Microsoft, GSuite, EMR)
- US work authorization
