Medical Social Worker - LCSW or MSW | PRN | Excell HH&H

Assess the psychosocial status of patients and families regarding terminal illness and environment, then communicate findings to the registered nurse and interdisciplinary team. Conduct social evaluations in the patient's residence to determine if alternative plans are required, followed by counseling on stress and coping difficulties. Maintain clinical records, provide referrals for practical and environmental needs, and serve as a liaison between patients and community agencies. Participate in interdisciplinary group meetings, quality assessment performance improvement teams, and the development of individualized plans of care. Maintain collaborative relationships with organization personnel and develop contracts with public and private agencies.
